This offer includes a full mouth examination, 2 x-rays, scale and polish, fluoride treatment, photographs and a treatment plan if requiring further work. This offer does not include any further treatment which may be required. Results may vary. The $199 is a fixed fee for this promotion and will not be adjusted if you do not require or decline some of the services included in the promotion. If you have a private health fund: your fund must pay a minimum of $199.00 for this service. You will be responsible for any fees up to this amount if your fund does not cover this amount. Any additional amount paid by your health fund is dependent on your fund fees and is not reimbursable to you, in cash or as a credit, or to your fund.

This offer does not include the initial planning and x-rays along with any bone grafting if required. The offer does include a limited implant consultation free of charge.
